Since February, My Ability Pathway (MAP) has engaged SafetyCulture for incident reporting and management, streamlining processes specifically for NDIS management. Designed to ensure efficient workflows, SafetyCulture’s system allows incidents to be reported, investigated, and appropriately escalated to internal and external levels as required. The platform emphasises a 'management' focus, facilitating real-time tracking and actioning of incidents and supporting compliance with MAP policies and procedures.

Incident Management and Actioning System
SafetyCulture’s platform allows MAP staff to report incidents through an easy-to-access app. This application integrates a custom template matching MAP’s internal policy, enhancing consistency in documentation. Once an incident report is filed, team leaders (TL) receive email alerts, prompting them to investigate and assess if an incident is reportable to bodies like NDIS, Safe Work SA, or guardians.
Streamlined Workflows and Tracking
SafetyCulture’s dashboard provides an overview of each incident, simplifying the process of tracking reports, investigations, and any subsequent actions. The platform's colour-coded progress tracking system enhances visibility, making it straightforward for any administrator to check on the status and completion of tasks.
Automated Reporting Features
When an incident or concern arises in relation to clients, the app prompts for detailed completion of the incident form and then flags for the appropriate team members. Notably, the system enables automatic link generation for reportable incidents escalated to the General Manager (GM) or relevant authorities. The digital timestamping feature allows authorised staff to access the latest updates seamlessly, even if another team member (e.g., the usual contact) is unavailable.
Heads-Up Communication and Follow-Up Actions
SafetyCulture enables MAP to manage "Heads Up" notices, providing insight into which team members have viewed and acknowledged communications or completed training. Timestamped logs ensure transparency and accountability.
